Blackheart is the son of Mephisto, who is basically one of the top devils/demons of hell in Marvel comics. Mephisto and Blackheart are both primarily Ghost Rider's enemies, but they usually mess with other heroes too.

@samusmex mephisto used the power of the 'negative zone" to create a being of pure "negative energy" which was Black Heart if my memory is correct, Black Heart earns the name due to his heart being composed of negative energy.
