His method breaks if there are 2 correct answers side-by-side or two incorrect answers side-by-side. In that case, when he switches answers, he won't know whether they're both correct or incorrect. He'd have to either switch both on, or switch both off to see, and that could throw off his system.

I think his method had to do more with there being 2 correct answers on each row. If the answers were distributed in a manner other than 2-2-2 (of which there are 4 possible distributions: 3-2-1, 4-1-1, 4-2-0, and 3-3-0), then his strategy would have failed.
Now, when he switches from C to D, he still has three right. The important thing to note is that he isn't totally without information. When the number doesn't change, he knows that both answers have the same truth value; either they're both right, or both wrong.
The contestant can use that information along with single switches increasing or decreasing the number of correct answers on the board to eventually find the correct six.

He got lucky with that distribution of correct answers. Had the distribution been like this:
NNNN
NNYY
YYYY
Where N=Wrong answer
and Y=Correct answer,
and if he continued the pattern that he was using, he would be stuck with three correct and three wrong answers for the entire game.

Now suppose that the answers are G, H, I, J, K, and L.
Let's say that he chooses the middle six in the above board. When he chooses B, C, F, G, J, and K, he has 3 right.
