Yes, basically in the book the successful bands of survivors were the amoral who would do anything to live- including eating people.
The skinny guy is from a particularly messed up scene. Basically people were being captured and eaten, but not all at once- the survivors would chop off legs or arms, cauterize the wound and then come back later for more.
They aren't 'evil' perse, more like utterly deranged by the horrors of the apocalypse, predatory, cannibalistic (there's nothing to eat). The squirming guys are being kept alive by the cannibals and the reveal censored out of the trailer is that they've had their limbs hacked off. Highly recommend the book, it's really dark and beautiful. Much more so than the trailer hints at.
I honestly recommend to anyone enticed by this trailer to read the novel. It is nothing short of a modern classic. However, I really have to question the decision to push the trailer in the "thriller" genre direction. The book won a Pulitzer, and if the movie is half as good as the novel this film will be a contender for Best Picture.

Supposedly the producers have 'upped the peril' of the book, naturally. Still this director is awesome and The Proposition was the closest thing I've seen to Blood Meridian done on the screen, so I'm hopeful this will be a good representation of the book.
They know they'll get the highbrow audience with the reviews and the word of mouth, and are pushing this for the folks who don't really read books, I bet.
Gotta say I winced when I saw that arrowhead emerging from the window...the most painful scene in the book...
What kinda bugs me is them tying down what the catastrophic event was, and using mostly stock footage of destruction.. Like you said it is probably meant to pull in unfamiliar people. Regardless the book was almost perfect ,adding things is not needed at all.
Scary freaking movie! Viggo Mortensen and Kodi Smit-McPhee(The boy) is breathtaking in their acting.
But its a not a nice place, the world in this movie, with some very scary people running around. Lord of the flies springs too mind.
