You have to download the Hammer of Infinite Fate add-on, then go to the Motor Forge and buy the Razputin head for Mt. Rockmore. (in case you don't know who Razputin is, he's the main character of Psychonauts, Tim Schafer's previous game) Then you have to put four of those Raz heads on Mt. Rockmore, and then you'll get your achievement/trophy.
