@bottledjeanz It's due to the uniquely crappy playback of lolreplay. It doesn't refresh ultimate cooldowns properly until they're used after fast forwarding to a particular time.

@Amberseal Miasma should be ranked last imo and ive seen that most great cass players do the same. Its very mana expensive and most enemies avoid stepping on it early game. It can harass to an extent early on but noxious and twin fang combo does a much better job. Late game its great to land your ult while the enemy is on top of Miasma.
