@innocentpie except, timelords can be given additional regenerations. remember how river gave up her remaining regenerations to save the doctor in "lets kill hitler"? well, if we assume that mels was her 2nd incarnation (say she regenerated into mels from the little girl) or 3rd (since the time jump is somewhat high, to go from the girl to mels being amy's childhood friend) then she might have given 6 or more regenerations. it's possible that the doctor was given more "continues" via this.
