If you let it heal completely over time, then you cannot remove the arrow. It stays there the rest of the game.

I had one stuck up his rectum and to me it wasn't a pretty site. To make it funny/awkward I didn't play for some time so it was naturally healed and I couldn't take it out. I think thats why they call him "The Fear" when you fight him

I took a couple bolts to the head the first time I fought the fear, then i saved the game without pulling them out, and when I played again, it glitched and he had them permanantly sticking out of his head. Even in the cut scenes. I would go in the medic screen and it wouldn't give me the option to treat them. I played the whole game looking like "Pinhead"
