What I believe James Horner was imagining when he wrote this song was...basically the passion of Jack and Rose. Their love for eachother, the power of their first kiss. Basically that when Jack died, his heart went on with Rose and at the end of the movie, when they got married on the ghost of the ship, it was there hearts that went on and joined as one.
