I'm taking a guess, because this thing totally had me braindead. So Alex and the girl is Phil and Ted's friend, and Phil caught her cheating with the guy with the newspaper (Clue is that he wasn't wearing any wedding band.) In the beginning, Ted says I wouldn't do this if I were you, he's referring to him breaking up the marriage by confronting her (and possibly Alex later) about her cheating. And then in the end, when he sees the actual lover, he says "You sure (you weren't expecting this?")
