My guess is that most adults and most adolescents -- perhaps even a few younger children -- thought this was probably intended as a subtle sexual reference of some sort when they saw it. By the time I reached second grade, I knew about all types of sexual positions and activities from many of my class mates. I understand about how kids know a lot more than people think, but then there are still those who don't. So it differs.
