Lisa had a drug addiction and Dr. Kaufman was enabling her. He would make her do the chores at the hospital no one wanted to do. He would even trade her drugs for sex. When Alyssa became hospitalized, the entire staff at Alchemilla knew there was something different, even scary about that little girl so no one wanted to take care of her. But Dr. Kaufman assigned Nurse Lisa Garland to take care of Alyssa. When she refused, he threatened to deny her drugs. She was to stay with her night and day.

Lisa had strict instructions not to leave Alyssa's bedside, "I have the feeling I'm not supposed to leave this place." Even though Alyssa is 'dead' by this point, her spirit still exists manifesting the world we know as the 'Alternate'. But when Lisa realized that Alyssa was now dead, she had no purpose in Silent Hill. Realization is the demise of many SH characters.

She killed her when Lisa walked into her room in the hospital to see what she was liked when she was burned, and allisa brought her back into the hospital afterwards to show her what she had done. Once she did see, she supposedly died as shown in this video. Or at least I think that's what happened lol.

@TheJohnnycheese That is the movie's version. The Alessa of the movie and the Alessa from the game are two different characters. There are many theories as to why Lisa was killed. I believe that in order to seal Alchemilla Hospital into the Otherworld, Alessa placed the Virun VII Seal in Lisa. When the seals around Silent Hill began to deteriorate, so did Lisa's body as she herself was the seal.

By the way, notice that she can't stand Harry's flashlight. That's why she runs back instead of embracing him, confirming that she was always a "monster."

Konami keeps hinting that she's still around during the events of Silent Hill 3, being tortured by Valtiel in the Otherworld. That thought is immensely depressing. How was she a bad person?! Valtiel is an a-hole.

@magnus378 Lo que pasa es que no lo explican allí. He buscado info de varias partes (wikipedia xD) ya que no entendía un carajo de lo que pasaba en el juego. Entonces entendí que todas las cosas que pasaban era causados por Alessa: el sonido de alarma era el de la ambulancia cuando fue quemada y la llevaron al hospital, los monstruos pájaro eran ilustraciones que hacía y los monstruos del cole representan a los niños que se burlaban de ella.

In a sense Lisa being all ready dead doesn't make sense. Lisa is alive in SH Origins while the monster"The Butcher" is around the town in both worlds till Travis kills it. The last we saw Lisa was her in a Motel room with Kaufman having sex and possibly doing drugs...yet we see no indication of some kind of injury from Lisa being killed in neither SH game and yes I've played both. I know both games were made by different companies but held continuity so it still strikes me odd at this scene.

it IS rather curious....one thing we have to remember is that this is the Otherworld where things go crazy, but Lisa really did transform. Alessa was making them in Silent Hill 1, but she wouldn't have changed Lisa because she was helping her(she doesn't know about the drugs).

Their are alot of breaks and moments in-between the visits, and we really don't know the full history behind Lisa actually; as the series progresses, she gets more and more unstable. Kaufman's drug-withdrawal threats, her torment in tending to the scarred Alessa, the appearance of the Otherworld and all the monsters...

I think the easiest and best explanation is that Lisa simply and finally crashed and lost the last of her sanity under all the pressure and events, inexplicably transforming.

@jpd2009RV I can agree, again to an extent. She did try to leave, but maybe it was out of selfishness and wanting to abandon Alessa. But the drug addiction got the best of her and she came back. Whether she was caring for Alessa out of the kindness of her heart or maybe she was just doing it as "her job" because she couldnt get her drug supply refreshed is debatable. We may never know her true intentions and if she had a good heart or not or she just was a coke whore.

That scene is still hard to watch...so much sorrow and sadness. It's things like that which makes the Silent Hill series so different from the other survival horror games; The emotional side of it. Yeah, spooky locales are spooky, scary monsters are scary and dark stories are dark, but the characters in these games really draw you in, make you feel what they feel.
