You can find all of the "old" millionaire music cues and sounds in various places over the web. In the US, though, as of September 8th 2008, all of the music for the show has changed. I remember a few years back however that Celador released a sound-track to the show on CD will all of the original music. You can probably still find it on Amazon or eBay, but it won't be cheap I'm sure.
It hasn't happened, but like it's stated below, the US uses the same core music from all the other versions. Not to mention the retired Disney World attraction "Who Wants to be a Millionaire - Play It!" had the same music package from the US version, and I witnessed first hand someone losing on the million point question, and that's the music that was used.
The US version uses exactly the same music/graphics package as the rest of the world. They wouldn't change a single piece of music that's used as rarely as this.
That said, in the UK couples version, the ordinary loss music was used when a couple got the £1m question wrong, but that question turned out to have the wrong correct answer, so they just had another question.
