Word flubs happen when you are talking extempraneously, just watch any sports event. Even with teleprompters there are mistakes. The questions are not discussed beforehand. And yes, we use notes to remind us of points we want to cover, just the way that Letterman, Stern, Limbaugh, Savage, and King do.
