The man launched a woman's head from his mouth, he would have said, "It's a man's life of launching woman's heads from mouth." then the colonel throws the switch and the woman's head flew away and the rest of the season 4 opening is now history.
Yeah, he went off to do Fawlty Towers with Connie Booth. When the remaining Pythons used any of John's old material, they included him in the credits.

In ESMR's Monty Python Season 4 Episode 4 - 4 paused at 02:49 the credits says:

J. Cleese (No relation)
(Of Hamlet's, that is)
the Hamlet-thing was the whole theme of the episode, which should explain it.
