The notes AND the timing are programmed. Notes: Obviously, just try to match positioning to notes (e.g. at 1:33). Timing: The beginning lets you think that the timing of the notes is determined by his balls (;P). But...

1. How does the keyboard know when to start it's background song?

Does not look fake to me. Notice that if it was a pre-programmed piano the balls would have to hit EXACTLY at the pre-programmed pace. I did not observe a single note played when a ball did not hit the keys, not even at the end. Each note hit when a ball hit. If the piano was pre-programmed it is impossible coordination.

No, it is the way that it works. When you strike any key that piano plays the correct note: With the balls he just says to the keyboard play now. Try to listen the notes and look at the side of the keyboard where it is hit by the balls

Very much fake, unfortunately. I agree with the person that said it's a sequencer. The last two notes right before he says to the guy "You don't have no socks" are the same note, but one lands lower on the keyboard than the other. Still, he does juggle well. :) Technically, he's drumming, rather than actually hitting the notes.
