It's to rest his head on, it allowed him to have his head raised from the floor which helped simulate the illusion that he was upright not laying down.

It's not hard. He puts his hand behind. Makes sense, because theres nothing underneath it. He never puts it in front directly because that's where the string is. He can easily put it on all other sides, cuz theres nothing there.
