Stuff can happen in ANY car of ANY age, brand, model etc. The FIRST thing you should do is use your brain. Definitely, shifting into reverse or park at high speed is NOT the way. The first thing is to turn off the engine (via key/button) and put the transmission into N. Then, when mechanical (mat, pedal...) and/or electronic (engine, wires...) reasons for unwanted acceleration are eliminated, you can apply brakes more efficiently than under full throttle. This woman's problem was the DEALER!
It is possible that Toyota/Lexus did not properly inform the dealer about the necessity to remove summer mats before installing winter mats. However, COMMON SENSE should tell the dealer that when there are HOLES in the winter mat that cannot be used when the summer mat is still there, it is necessary to remove the summer mat and attach the winter mat properly. And it is true that the brakes should always overpower the engine. Try it in your car (ANY brand) to make sure the brakes are OK.

There was a highway patrol officer where I live (San Diego, CA) who was driving an '09 ES350 loaner car with his wife, daughter, and brother-in-law. The gas pedal jammed, they hit 125 mph and they eventually crashed into a Explorer, flipped, rolled down an embankment, and blew up in flames. All 4 people were killed. This is a true event, and people have died from this.
Toyota can consider themselves lucky that the black box burned up. If the truth was ever revealed about an obvious computer glitch in its' cars, Toyota would close its' doors forever. That will never happen however, since Toyota has owned consumer reports for years, and is somehow connected to NHTSA. This womans' story is incredibly similar to the 911 call made by the brother-in-law that died in that wreck. Sorry, folks; too much of a coincidence for me.
These cars are defective and are killing people all over the nation and Toyota is using their chokehold on the country to sweep the problem under the rug. They've even issued a B.S. recall blaming the floor mats to make it look like they care. Guess what. They DON'T.

They are the most reliable and safe luxury cars that ever hit the road. It is a lady driver thats why shit happened to her. I hate when women and crappy drivers take nice cars to the road. I got a 93 Ls 400 with just under 300 000 clicks on the odo. and not a problem at all.
