You can get a similar page by going to:

1) blootow . com

2) Click on FREE website Templates link on left side

3) Press the URL ICON button

4) Press Generate FavIcon button

5) Once you see the HTML kit site then hit browse to select your image and follow the same process I used from there...PM me if you need more info than that.
