Actually itchy did stay in heaven. In a rare episode It's revealed that Itchy volunteered for 20 missions Because he missed Charlie and after that he has the option to return to heaven or volunteer for 20 more.

Maybe because they used Charlie Sheen to act the voice of Charlie in ADGTH2 and then he didnt want to carry on so they got Steven Weber to do the rest.
