You can clearly see his head (only his head) whipping back from the hit, he then has his hand raised to his head after that. If you got hit in the arm or armpit, why would you start holding your head? Everything about the entire play (during and after it) suggests he was hit in the head. The medic is immediately observing the pitcher's head once he reaches him.
