An interesting aside re: copying costs: HIPAA requires both "access to" records, and "copies of" records. They have 30 days to produce. They get to charge for copying costs, but NOT to provide access.
What I've found is if if you make a HIPAA request for "access," the PITA factor is such that they'll generally just give them to you and waive the fee.
