Actually the "Kip" and "K-toss" also known as the "K-flip" are two completely different aerials (i know that the same aerial goes by many different names but those two names, as far as I know, have only been used in reference to one aerial). Anyways the issue with the attempt in this video is actually an issue of momentum. She needs to continue heading past you a bit to get a good arm connection other wise the guy has to create a forward connect out of nothing. Also your hand hold is wrong.
Bicep to armpit -- kind of, depending on who teaches it, it's typically on one side or the other of the bicep. That bicep/armpit connection is the point of rotation regardless. Try to find a video with a smoother entry, there's a lot of "down" in your prep before the toss and I think you're losing energy there.
When my group does it, we call it the boomerang- if you still feel a little uncomfortable with it, try having more connection up her back as you're actually flipping her- it's hard to describe in text, so i don't know if that helps... good luck with your stuff!
