@tmac9938 What happened is he climbed too high on the ladder, and had nothing to hold on to with his hands. The ladder didn't move. His fall had nothing to do with the slippery floor, and someone holding the ladder would have just gotten injured too when he fell. He did miss the rung when he started to step down, but if he were holding on to something with his hands, he would have been able to find it with his foot and descend normally.

Second, he climbed up to the point that his waist was above the top rung, which is a ladder safety no-no, as your center of gravity becomes more mobile. Third, he reached overhead. All three factors combined to shift his center of gravity behind his feet, so a fall was inevitable. "Slippery floors" had absolutely nothing to do with it. In fact, the ladder hardly budged!
