This story is probably about kidnapping and murder of a little girl. Before "Maachan no Ringo" was finalized, there existed another unfinished version which had Maa-chan trying to send a letter asking for help, but the green guy ate it. And she was pushed of a bridge into the sea and drowned.

About this version, I'm not sure what having her head being replaced with apple means. Either she also really died, or she lost mental capacity, either way the green guy gets to live with her forever.
