The Joker's make-up isn't always on. Did you not watch the 90's animated series? In 'Mad love', The Joker's make-up is off for a bit, due to him being in Arkham Asylum. It isn't until Harleen Quinzel becomes Harley Quinn that the Joker's make-up comes back on. Sorry, but I had to correct you on that very simple fact.

Also, Christopher Nolan wanted to create a more realistic approach, while Tim Burton kept with the comic series' approach. Both are brilliant, in my opinion.
