My stepdad had a Remington 700 that would do something similar. There was a problem with the trigger mechanism and when the bolt was closed on a round, the firing pin would drop, causing the rifle to be fired without input. This looks like something similar.

I watched a guy take a charge 6 from a 105mm howitzer and all it did was knock him out. His glove got caught on the firing mechanism while switching it from safe to fire. Its survivable. Generally you will be pretty fucked up but its not as bad as it looks.

Depends on what type of initiation.

If it is electric like the British Light Gun, a stray bit of static electricity could have set it off.

If it was the firing mech - as you open/or close the breach the firing mech cocks itself - the firing pin could not have engaged properly due to worn sears or springs etc.

Or it could have been damaged, or degraded ammunition. This is caused by poor storage conditions, or careless handling or even tampering.

now THAT is fucking dangerous. My grandfather knew a man during WW2 who was part of an artillery crew and opened the breech too soon. The spent casing shot through his stomach right through to the other side, damn near ripped him in half. Needless to say, he didn't survive.
