The culture on the MTR is that people do give priority to the elderly, pregnant, etc. The MTR in Hong Kong runs every FOUR minutes (on most lines), their system really is quite efficient.
One thing to note, in Hong Kong the trains run around every FOUR minutes. Seeing a train this crouded that runs every four minutes shows nothing wrong with the corporation, just the mass population. Their system is very efficient (I'm an Australian living in Hong Kong).

@deany1980 In some lines, they runs every 1minute 45 seconds during the rush hour. This is properly the highest capacity of the system.
