I can think of another. This small ships usally only have 2 navigators. Going watch 6 h on 6 h of. And thats only the watch, with extra job you can easyly get up to 16 h / day.
Finally the job has run the officer on watch into the ground and he falls asleep on the bridge. with only 2 AB (asleep),their is no lookout to wake him up. The easy way is to blame the OOW. But the problem is so much bigger then this. As long as we have ships with only 2 navigators we are going to see stuff like this.

It is actually a quite common practise. That way the ship will not drift off ground again in gcase off hullbreakage wich again could lead to the loss of the vessel.
