Teron Gorefiend was the first death knight created with the soul of a necromancer placed within a dead stormwind gaurd even when you fight him in black temple he says "You know I was the first!" he's referring(correct spelling?) to him being the first death knight and Ner'zhul is the first LICH KING he soul and mind were placed within Frostmourne and the helm of domination(Arthas' Helm)

Turns out Bolvar Fordragon shall be the new lich king as predicted by Uther. There must always be a lich king. Look it up MMO-Champion reveals it.... Though I hear the Lich King's death shall be among the coolest cinematic in the game!

I guess the explanation is offered in the Halls of Reflection. Uther is talking about why there always has to be a Lich King, and that he believes that there is still a piece of Arthas left within the Lich King that stops him from doing just that.
