Not even close to perfect. Bring your ass down below parallel and focus on the hip drive - that's the whole purpose of the squat. You need to tighten your lower back and keep your chest up. Your feet look OK. Gotta focus on bringing your ass up and out on the squat. If that causes your upper body to tilt forward slightly on the way up, that's fine. It looks like the bar could be lower on your back.
oh and to the people saying its the worst form ever are quite obviously simple. just yesterday i saw some juicehead load up the bar with 600 pounds and do a range of motion so bad i was laughing out loud. bad, yes? the worst ever? nope.

Overall form is ok, back looks straight, knees look pretty good and sitting back. Depth isn't there, "parallel" means the crease of your hip goes lower than your knee, not the angle of your knee is 90 degrees. Take some weight off and go lower. Also the spotter should be spotting the bar not humping you from behind.
I'm glad that you are squatting rather then doing obsessive bench pressing like most young kids but you need another 8-12" of depth. If you get used to half squats like that it could mess up your knees later on.
