Here's 2 possibilities that could've happened:

1) He was hallucinating since the effects of the marker had taken over him

2) She was real and he killed her before she could bite his head off

i wonder if isaac is still alive i know he took his helmet off but if his reactoins were fast enough and he still had his uit on he may have survived the ending didnt actually show him getting killed so theres a possibility he may be alive but jeez getting murdered by someone you love who mutated into a monster wow what a way to go...
