Your hips are swaying too much. You cannot swing like that on the course because you are anxious to make that same swing from your backyard.
Instead, you should relax and take your time through the swing like you do from home.
You could make believe like you are in your backyard and forget the golf course. That in turn will relax you and you'll take better swing.
Last, concentrate on a routine to relax you.
