To my speculation at least from what I know. (Hell I could know jack shit). I think Obama giving money to the private insurance companies was a "logical" choice. By doing so he would have given better coverage to those under those companies or at least made it more affordable. But here's the problem. They didn't. Obama is not an idiot. But a fool. He has faith that he can appease the whole population, to work with the republicans, and for people to make the right choice. What he needs are balls.
Working 6 jobs at 6 different McDonald's doesn't mean you should earn more than that man at the top of the hill. That work is much different than the guy working jobs it takes a lot more risk.

Interestingly Americans pay the 2nd highest (per citizen) in Taxes for HC in the world, only we still have to pay out of pocket.

You're usually pretty spot on, but this is just blind envy. Gates/Jobs weren't birthed as CEOs of major companies, and their businesses don't exploit anyone. The real travesty in this country is in the siphoning of productive capital into political projects for destructive agendas which target the poor for victimization. The less money in Gates' pocket, the smaller his business, less-effective his products, and higher their prices. Now that money builds prisons for non-violent offenders.

Medicaid doesn't provide care for all poor people, each state has it's own version of it. For example, in Arizona you have to have children and make so little that you can't shelter or feed them. So if you're single and make 20K a year you're S.O.L. Your 2nd point is correct, in fact an individual mandate is something the healthcare companies want. Interestingly Americans pay the 2nd highest (per citizen) in Taxes for HC in the world, only we still have to pay out of pocket.
