I work in a daycare, and poo "explosions" do happen. Not to this degree (obviously) but they sometimes will just seem to explode out of the diaper. All down the legs, up the back, up the front. It's gross. And it's because the kids will poo, then keep playing/sitting in it. It just gets smushed around. Nasty, but a fact of life.
