The shootout at New Otherton isn't unrealistic. Remember, Keamy's team have Miles with them; Miles was present at the little meeting that included Sawyer saying they should hand Ben over. Miles undoubtedly told the mercinaries this. The point of killing everyone around Sawyer is to induce him to go get Ben and hand him over, which is exactly what Ben says when Sawyer finally gets into Ben's house--and indeed, Sawyer continues to insist they hand him over even after Alex was killed.
