The easiest way to diagnose the problem: Check if you have any muscle spasms in the pelvis area, usually piriformis muscle goes first, later all other muscles like: glute max, med etc... Bc if the sij is not stable but kind of in place it's not easy to see anything on MRI or xray pics, moreover lots of docs dont believe in this problem at all. Any instability creates muscle spasms to stabilize the joint naturally, which is not enough since ligaments are meant to hold the joint. Moreover un
Consider this is the best advice from real neurosurgeon from GB:
Adjust sij and let the ligaments heal, usually it takes up to 12 weeks resting on the firm bed moreover, get the Joint supported with the belt. We were able to restore that joint in ~50% cases without surgery. SIbone surgery is the best fixation nowadays, fixes the problem instantly. If not fixed, this will kill the whole spinal cord finally leading to spondylitis disc degenerative problems
What qualifications does this person doing the adjustment have? The reason I ask is that his clinic looks very crude, and this chiropractor is doing the same adjustment watch?v=NhwECwj3LDI&feature=related but its done differently, and a lot less crude looking that this video.
What the hell he had to go to all those doctors. Hell my chiropractor does those same tests with the heels every time i go in. Seems like the docs aren't very good in Europe. Everything he did was basic chiropractic skills.
I just found out yesterday that I have uneven pelvic bone,,my chiropractor is giving me adjustments and I hope he can make it even again :( Ive been living with low back pain for sometime now
Ok, my doctor actually put the right diagnosys long time ago BUT he change the position of hips one day but gradually it came back to the same level :-( It's very dissapointing... does everybody have the same experience? If the doctor on video can really treat it more professionally, can you please advise? write to e.a.sokolova@gmail.com
I've had this exact problem for years with right side pain. I too have been to serveral doctors and chiropractors. They say nothing shows up on my X-Rays. I was diagonsed with SI joint misalignment by a sports doctor but he only referred me to another chiropractor. Nothing has worked for me. I am constantly in pain all the way up my right side from my hip to my shoulder. What kind of of doctor should I see to do this kind of adjustment. I live in Canada. Please help!
This is exactly what my problem is for many years now, sadly docktors in Norway are useless and unable to help me :-( I wish I could go to this docktor for help....where is the clinic?
is there any one in cornwall who could help me please i have both my ribs are hurting and around my back, think its from slouching sitting just need some one to push my back back in,i dont want to go to these expensive chiropractors, been there years ago if you can help leave a message thanks as havent got mutch money
This video fails to address the reason for why his pelvis is out of alignment. The reason is muscle imbalances. Everyone is different, what is shown in this video will not work for everyone. You need to find the muscle imbalances that are causing the problem and perform the corrective stretches and exercises for the specific muscle imbalances in your body. A website called Lose The Back Pain explains it better than I can. Good luck.
@parantunut "Re This video fails to address the reason for why his pelvis is out of alignment. The reason is muscle imbalances" erm do you honestly believe that physios and doctors understand the cause of back pain? Especially doctors, that's why they prescribe pain killers like Tramadol because they don't have a fucking clue. Physios are rubbish, they just prescribe the same old generic exercises as though one size fits all...lol
where is the scar of the surgery? remember he said he had surgery to correct the hernia! Also let suppose that this manipulation has corrected the problem and he becam pain free...but he does not say that the problem can happen again and often, or did he?
My point is: it will be nice to clarify that this is not a curative treatment. I mean it will be nice that during the video some general info will be exposed. For example statisticaly how often rezidive can happen etc?. In my view this way you will make the video more accurate. My point was not to doubt about the treatment. Thanks for taking the time and answering
I do apology i used the word recidive in german rezidive but this is the meaning of it: récidivant (adj) - recurring; récidiver - to reoffend, commit a second offense, recur; le récidivisme - reoffending; récidiviste - second/habitual offender; la récidivité - recurring nature (of a disease)
This practitioner is right on the money. Some minor omissions, (not his fault). Xray also showed pubic symphysis misalignment which was not apparently corrected. My way is with patient supine, but essentially similar. Helpful hint . . if you want to try this, find the SIJ dwell point first . adjustment is then more comfortable. Table tennis daily . . check recent advances in pelvic fascia; some news there.
Might happen spontaneously but unlikely. It pays to check, then use MET. If that fails, go on to more assertive moves. Pub symph acts as a lock for the pelvic ring, and force closure there needs to be modified at the instant of adjustment. cheers .
Learned it years ago in a workshop based on Rich Paigh. Innom manips alone are not precise enough for me in resetting the symph. MET uses the adductors to spring the symph, but only after doing the SIJ's. Always ready to test another way. Perhaps if you google migratory fascia in pub med, also look at Diane Lee's website, useful general stuff there too. Lots to read!
spondylitis...But the majority of problems dont link the SIJ....A therapist should treat and educate the patient by alleviating the MECHANICAL factors that bring the backache (from self-treatment exercises to correcting posture)...ONLY by this manner there will be a LONG-LASTING relief of pain...That is my experience in practice, and because this works almost always i dont give a damn for any "malalignment" in SIJ...just that, if u want to talk in your terms...
I do give a damn after suffering from it 15 years. And I have been communicating with many others who had the same problem and are now painfree without useless "treatment exercises" etc. that we did for years without any help.
By the way, quorthon...most of us in our 30's were too distracted from our own health problem concentrating on our hazardous job try to earn a living but ending with illness & ailment. Some even endup paying doctors money of hard earn money all our lives.
Wow--I've been reading this thread and am amazed at how defensive and closed-minded quorthon1940 is. I have an SI joint dysfuncation where my left iliac tilts anterior and the rigth posterior. When it is in place--absolutely no pain. When it goes out--pain. I found an amazing doctor who recognized the problem and his PT was able to put it back in place and showed me a correction when it goes out (because of lax ligaments). The pelvis does move! and a slight unilateral shift is all it taktes.
I am a NASM certified personal trainer and corrective exercise specialist. I know about biomechanics--the kinetic chain--muscle imbalances and ways to correct them. There may be an imbalance that makes my pelvis shift and I am working on that. When my pelvis shifts--my left leg seems up to 1" longer than my right. I am literally tilted until it's adjusted. I was told I have degenerative disk by orthopedic surgeon. Most people do to some extent. That was not the CAUSE of my pain. My SI joint was!
well because my friend u are a "personal trainer" and a "corrwective exercise specialist" (WTF is that??lol) u have no DAMN RIGHT to speak about C-L-I-N-I-C-A-L problems!! in my profession as a physiotherapist(in clinical grounds)we have the legal RIGHT to treat patients, u dont!! So simple dude and no more talking!!
and btw i am VERY close-minded!!! So much u cant imagine!!
wow--thank you for correcting me. I am a worthless person who has never helped a person because I am not a physiotherapist who can't spell and is extremely rude. I truly feel sorry for your "patients." You my friend are truly "S-T-U-P-I-D." Go back to your cave.
By the way--I don't "treat" patients--I help to prevent injuries by addressing muscle imbalances BEFORE they cause injury. I always refer if potential client comes to me with active pain. Before I studied for my certs (well-respected ones--look them up) I had a year of pain from impingement. 2 orthos and 2 rounds of PT did nothing to "CORRECT." I fixed myself through stretching/foam rolling pec major/minor/lats/levator scap/upper traps and strengthening rhomboids/mid traps/rotator cuff.
fix your brain too, through stretching/strengthening,I suppose that you will find many "imbalances" in there, trust me!!!....I am going primitive again!!!
since 4 "C L I N I C A L" professionals such as yourself couldn't "fix" me and I fixed myself I am quite happy with my imbalances in my brain. You aren't worth my time.
biomechanical factors (prolonged sitting, bending, loads on flexed position, etc) and the problem then is prolonged by many pscychososial factors that will lead to CHRONICITY of back pain....The main "root" of the problem is exactly on these factors....SIJ plays minimal role on that....Though i cannot deny that SIJ pain (u call it dusfunction or whatever) can occur in certain situations....Like pregnancy, trauma (fall on buttocks, blow on the low back) or inflammatory processes like ankylosing
u still focus on a certain field of literature....I can give u some links too by the studies i mentioned u too....In fact most of the studies that i saw dont include large samples of population and many of them are just "conclusions" from SIJ experts by applying a couple of manipulations....neither to mention that there is no FOLLOW-UP that will give us strong evidence that this theory is true...As i have said before the fact that many people suffered low-back pain is by hundred reasons from
If pain comes back after few days or weeks, you need to re-correction. If you don't re-correct but only follow, the results from one treatment are not that great. Most important is that people learny what is the cause of their pains and how to get rid of it.
by "muscle spasms" in the back which then can affect the whole system and and....They discard EVERY different aspect in a FASCIST way!!!My point is that this MONOLITHIC manner of thinking in such a science is almost catastrophing in clinical practice...Nothing comes from ONLY one source!!The clinicial should have means for proper DIFFERENTIAL DIAGNOSIS....Open your mind, read more books and articles, reconsider some thoughts and then we talk again my friend.
I agree. Muscles affect the whole system. Muscles are the factor that connect pelvis to shoulders, pelvis to thorax, etc.
I have read some studies about palpation and provocation tests for SIJ dysfunction, but because they believe dysfunction is pain in sij their results are useless. Some interesting stuff exists drom Cibulka, Hesch, DonTigny, Al-Elsa, LeBlanc, Schamberger and few others.
the field of musculoskeletal medicine is completely DYNAMIC, and beliefs that were believed 15-20 years ago can easily be abandoned if there is a steady scientific proof via research.... i know many people like u who have similar beliefs, for example one says that all BACK PAIN caused by "Instability" in the deep muscles of the back, the other claims that BACK PAIN caused by abnormalities in cerebrospinal fluid (check the word "craniosacral therapy" to see) and the other that BACK PAIN is caused
as i see my friend u live in your own reality....Your philosophy that EVERY kind of malfunction (back pain, sciatica, neck pain, headaches, what else carpal tunnel syndrome maybe???) is caused by "PELVIC DYSFUNCTION" give me the chance to advise u that you should start looking from a UNBIASED and truly SCIENTIFIC literature about the subject we talking....When someone claiming himself as a SCIENTIST and not an "experienced chiropractor" he must be fully aware of what is new in his field of science
I have learned that scientific way is not allways the correct way. Science don't believe pelvic dysfunction exist. Science don't believe facet locks exist.
Leas but no last...What i said before about the asymptomatic patients and a possible difference in mobilty i meant that simply this normal "variation" in "mobility" (i put " " because mobility is almost absent) means that this clinical sign is completely ATYPICAL and valueless in a clinical examination....I think now that i make myself clear....
This patient of course it is possible to have some sort "POSTERIOR TILT" in his pelvis (lets say that it will be more pronounced one-sided.) and of course the pelvis will be dysfunctioned....But think this.....All this DYSFUNCTION is merely a R-E-S-U-L-T of what had caused the back pain in the first place... and it is the bad posture, repeated bending if we talk about this patient.....So u as a therapist will u treat the CAUSE or the RESULT???? consider that before answering....
In this occasion if you target on a "dysfunctioned" pelvis, which is TRULY obvious u will make serious harm to patient....Instead u should focus what happened in LOW BACK!!! when u resolve that the whole system is getting straight...Maybe it is an extreme example, but i think u get the point....To make it more simple.....The classic presentation which is commonly seen in private practice clinic....Low back pain because of prolonged sitting or bending with often radiating pain....The same is here
"Most mechanical problems in back are to due to dysfunction in pelvis"!!!! Where in the name of god u conclude that????
Have u ever wonder that the opposite is more possible to happen??? A MECHANICAL (name it disc, facet, ligament or whatever u like) LOW BACK PAIN with all that accompanies can give malfuntion to the pelvis and lower body....An example on that...Think about people with "LATERAL SHIFTS", means that because of some acute disc pathology they become "inclined" to one side...
As a clinician i examine the SIJ lastly after i have exclude MECHANICAL BACK PAIN (Check the McKenzie books for this rationale...) then the HIP and only if those are not the source of pain (and consequently the "dysfunction" as u claim it) i would suspect the SIJ...I never trust palpation techniques but i prescribe exercises to the patient which truly "mimick" movements that u can achieve by mobilization (and not THRUST manipulation!!)....
Many doctors diagnose the dysfunction with Fortin finger test: A patient points the location of the pain. If he/she points to SIJ area a doctor may diagnose dysfunction to that place. most doctors don't much investigate or touch the patient.
Most mechanical problems in back are due to dysfunction in pelvis. It is easier to first correct pelvis because immediately many back muscles start to relax. It is easier to open facet locks and other locks from spine after pelvic correction.
Not to mention that many ASYMPTOMATIC people has "differences" in SIJ mobility...
u seem to be familiar with the term "DYSFUNCTION"....when one tissue is normal, suppose it is the sacroiliac, and i mean "normal" by the fact that it is NOT the source of PAIN, that the patient is complaining off.Why sould this tissue has "dysfunction" when it is not the source???
Sure. Like I said: pain comes usually to somewhere else. More than 80% of all people have sufferen pains because of pelvis misalignment/twisted pelvis.
I can give to u as many proofs u need about the fact that SIJ has MINIMAL movement, u just check the work from Sturesson in the literature(i can give u the exact articles if u want...) where he proved by a stereometric alanysis that 1. the movement of the ilia on the sacrum is about 1-2 mm and the most important 2. people with back pain DIDNT have HYPOMOBILE or HYPERMOBILE sacroiliac joints especially if the pain was one-sided!!!
and is entirely an "neurophysiological" phenomenon (if u are a bit aware of the -so called- ARTICULAR MECHANORECEPTORS and how they get affected by external forces and how they inhibit or exhibit pain, u wil probably see my point). Finally i want to comment that methods like palpation and "estimating" anatomic landmarks as u did on video is by far an UNRELIABLE method of examination, also proven by many clinical trials and studies....
possibility that this guy had a HIP dysfunction had u checked it??? Definitely no!! The fact that this guy has instant improvement says nothing, because it is impossible to MANIPULATE a joint (SIJ) that has 1-2 mm movement as it has been proven in the literature....U should show us a FOLLOW-UP video of your patient and see if that improvement was "permanent", which i doubt cause in fact NO movement has been made (as i stated earlier). It was just the usual effect seen after a "successory" thrust
a critical question to the guy who uploaded this....by what kind of examination u figure out that the pain is sacro-iliac related??? let me remind u (of course if u are a registered physiotherapist or chiropractor) that the sacro-iliac prevelance in back pain is 15% according to literature (see works by Scwarzer, April, Bogduk and many others..)...Only if u have differentially diagnosed that there is NOT MECHANICAL LOW BACK PAIN u can see other possible resources of pain...for example the
Are you used to running on the right side of the road all the time? if yes its the reason - one leg longer ...you should run one time on the right one time on the left ...if you run in the park its not this.
This patients Xray pretty well matched my own. Right hip slightly lower than the left causing a slight twist of L5,4 & 3.This in turn slightly bulged a disc, and including a slight twist of the spine was causing all sorts of problems with nerves under pressure, causing spasms and leading to major sciatic pains.Relaxant drugs are good to free the spasms so the chiros can then adjust the joints and then work on the soft tissue areas to help keep things in place. self awareness is key too.
this video really helped with understanding MY joint dysfunction. i am a massage therapist myself and have the opposite hip higher than this guy's. it was caused by an injury as a child- and now at 25 years old my symptoms are arising.
hello will you please advice me in regards to my moms accident and resulting to spondylosis in the lumbar spine, in this case is she may undergo surgery or what?... because in the summary of impairment of my mother it was stated that" she will be placed in a DRE Lumbosacral II and award her a 5% whole person impairment for asymmetrical loss of ROM of her Lumbar Spine as well as palpable spasm"... What does it mean? I hope that you can help me on this i'm willing to give more info about it...Tnx
I have been having the same issues for almost 5 months now been to a chiropractor that did the infamous roll technique and I am now in physical therapy. I have been having pains in my lower right SI joint my gluts hamstring knee and both hips. When i walk it feels like the level ground I'm standing on is uneven. All of these treatments and I am still not better. I wanted to know more information on possible corrections of this type of injury.
If you have low back pain, mid-back pain, etc., there is a really good book called "Healing Back Pain" by Dr. John Sarno. It conveys, without a doubt, the most Responsible Content on the matter that I have ever come across and is NOT "about" anatomy in general. You can google his name. You can probably find it at your local library if you don't want to purchase it. P.S. I've had 2 discectomies in the past - didn't know better, and.....yeah, it's a really worthwhile read.
if you see a good physical therapist with a spine background (most outpatient clinicians are) and you can get the adjustment and they will strengthen the muscles so it does not happen again
Do u guys ever look into research or are u happy w/ assumption therapies? So easy to fool the public with parlor tricks proven by science to be invalid. Not trying to be insultive, but how come u guys are so disinterested in valid research? When u pushed on him, it may have helped him because of extension in spine;not because of changing symmetry, but we don't get to see the post Rx response. U just try to say he's now straight; who cares,he's fighting gravity;even if you could straighten him,
Go see a good chiropractor that does full body adjustments. Doctor's and inexperienced chiropractors (ones that show x-ray and say that they cant correct you and such) is BS. My chiropractor fixed mine and my one leg longer than the other. Find a chiropractor through knowing others who have seen a chiropractor, pilates teacher (where my recommendation was from), etc. Recommendations is the key to a good chiropractor to make the best of your health and money.
There is no way how you could know a good chiropractor in our country. The film was taken in Finland. And anyway a chiropractor is useless as in this video you can see... and a chiropractor couldn't help him. He already tried.
4 years ago, i had an amazing problem with my lower back on the right site. After several visits at docs in germany i've decided to listen to my father. He suggested going to an old experienced man (in Turkey) who works with very old massage techniques!! First it seems strange, because he was not a academic doc! Then i stood in front of a very old guy, he was 97. Now, i was laying on the ground, 4 persons holt me on my arms and legs, an the old man graped my neck. First he looked after my back!
I am afraid my father knew his name, BUT this guy died 2 years ago! But in Turkey, there are some other as i heard. But i don't know them, because sinc i have been to this old guy, i had no more pain in my back.. (lowerback - right side) again!!! AWESOME!!!
He found a point, and then he said "oh it is non on the same line" i said that it was always like that! Than they told the others to hold me with all their strenght..and i was told to become relaxed! AND all i know is that i heard a big "KLACK" in the area where my belly is. After 20 minutes relaxing i get up and... wow i did't feel any pain or other bad things. Now 4 years later, i'm sill painfree. After this, i've made many experiences like that..OLD IST NOT BAD, STRANGE TECHNIQUES from asia!
Awesome Old man Techniques, Here i'm really feeling better.. SIJ problems are terrible i'm glad u fine.. I can't wait to see how ill be feeling by the end of the summer!
Guys, never give up, once we're all fixed we will have better life then others ;)
Im havin pain that runs from the buttox to the lower back and i end up on bedrest for 3 days. because of swollowen lowback on rightside sometimes left..as i seen in video i lean forward now more then standin straight up as i walk...i take smaller steps to walk because longer steps make me sway from side to side or limp.. when i try standin straight up or lean back, it pulls on inner thigh, near groins very uncomfortable feelin..
hey this is a simple chiropractic case, any competent chiropractor in the USA would notice this and be more than able to treat this condition. Wish you the best, just check for any PALMER trained chiropractor in europe and they will be able to help you..
That patiend had visited chiros, no help. So did I. Tens of other patients share similar experiences. They claim they know it, but very often they don't.
doing any exercises to stregthen opposing muscle groups? He will surely have Psoas and gluteus med. involvement due to pelvic torsion. Look for knee addtcn on a single leg squat =gluteus medius weakness from overstretch. check for ham weakness on eccen conctraction b/l (the ability to bend the knee while moving backwards, say in a reverse lunge. a tight, weak hamstring will continue to allow the high hip to pull up in combination with quadratus lumborum tightness on the ipsilateral side.
Something like that. It is a pity not all know that or even anything like it. Pelvis torsion creates muscle problems to piriformis, psoas, gluteus and to many others. After adjustment of pelvis the muscles have to be stretched to adapt.
Problem is that just "opening the SIJ lock" is not enough to everyone. The ilium can be lifted up some sentimeters and opening the lock doesn't move the ilium back to it's place.
And anyway opening the lock leaves SIJ very loose. It must be pushed back.
whenever there is a high ilium, the L5 lumbar will rotate toward the inferior side. L4 will typically rotate toward the high side unless there is thoracic or even c-spine involvement. you should take these into consideration when addressing any SI problem. also, just stretching a muscle may lead to more problems, you should check to see if the muscle is weak because of over-stretch or over-contracted. if overstretched, strengthen (dont stretch) if over-contracted, stretch (dont strengthen).
also, it looks by the way the therapist is "unlocking" the SIJ is by causing a myotatic reflex within the QL mm on the R. This will normally be reset when in standing position after a few steps because of weightbearing principles of tonic head/neck/pelvic reflexes that right the pts body by head and eye movements. Addressing the entire spine and chiropractically adjusting the SI, L5, L4, T13, and C1 will typically help solve the problem
> Addressing the entire spine and chiropractically adjusting the SI, L5, L4, T12, and C1 will typically help solve the problem
Sounds great! It is a pity that all chiros don't know that. Or perhaps they do but they don't know how to adjust them. Why that info is not published anywhere? You should make that information known to all patients!
Not to say that there are not congenital and anatomical reasons for hip misalignment, but lifestyle is a major contributor and makes it worse over time.
I know an athletic trainer that did a different procedure to correct the alignment of the hips and length of the legs. It involved putting the persons shoes under different positions on the back. It really helped me, but it is a procedure that must be repeated. The awkward positions we put ourselves during our daily routines and the shoes wear cause misalignment of the hips. Unless one changes his lifestyle, and properly designed shoes are made, we can not escape at least a small misalignment.
Leg legth difference is usually not anatomic but a symptom of twisted pelvis. A heel lift is a way to correct "length difference", but not the cause of the "leg difference".
Hey Man, there is a Doctor in Columbus Ohio, not just a Doctor, his also has a whole team of Doctors that together they fix you. I was also a walking draging my right leg dead person. Should have seen the pain I was in. Fixed. They have their own Hospital and they won't leave you in pain. Dr.. Daryl Sybert from Ortho Neuro in New Albany Ohio{hospital] and offices at 1313 Olentangy River Road. He will give your life back. The Doctor who other Doctors will see for their problems. Good Luck.
I dont think ppl in Canada/Quebec are qualified to find this. MAN I'M IN PAIN! It's been 2 years now. I've just got injected and according to the video i dont think i will see progress. I feel the torsion, when i crouch the he is doing i have the same behavior. Is there a way to fix this with threatents or exercises? Who should i see in Canada ? i dont mind paying lots :P IM a walking dead !

As a side note: at the end of the video there is a quote about physicians not being trained to find these kind of problems. Chiropractic physicians (and DOs trained in OMM) are trained to find these dysfunctions (and do so every day)
If it is known there why there are many forums where hundreds of patients have seen many doctor and chiropractors and still no-one has done nothing like that to correct their pelvic malalignment?
Sure some people have got help, but not all. Because not all chiropractors understand that problem.
And what was on that video is not taught to chiros. They have been taught to do it differently.
Actually, that is not true. In the USA, this is taught in the first year of their education. What is in this video IS what is taught as a very basic correction.
I have this exact condition today after falling on my pelvis 9 years ago from a bycicle. Can this be healed in one time like in the video? Did it really straighten out for good after this visit?
If you have SIJD your pelvis is twisted even if you don't stand or sit. Twisted pelvis is causing curves to spine even if you are lying. So it can be the cause. I agree it is not the cause for all of them, but for many or even most! Now doctors don't even believe that SIJD problem even exists or that it could be corrected with manipualtion treatment!
You continually post this on the National Scoliosis Foundation's forums, and give the impression that SI Joint dysfunction is the cause of all idiopathic scoliosis. It may be responsible for some cases, but it is NOT the cause of all of them. I have infantile idiopathic scoliosis and was diagnosed with very big curves as a six month old baby (62 and 40 degrees); the curves were there before I even began to sit up. That cannot be explained by any SI joint problems.
The same thing happened to me when I was 15. I had a herniation at the L3 and L4. My right leg appeared an inch and a half shorter when laying down. I actually went paralyzed for about 20 min because the joints were pinching a nerve. After a year of doing back excercises, everything is back to normal. Hope you stay well.
The easiest way to diagnose the problem: Check if you have any muscle spasms in the pelvis area, usually piriformis muscle goes first, later all other muscles like: glute max, med etc... Good luck.

>This video fails to address the reason for why his pelvis is out of alignment.
No it doesn't
>The reason is muscle imbalances.
No it is not. If it were that it would be easy to correct. Every physio and doctor would give it as a "description".
But they don't. Why?
parantunut 2 years ago
@parantunut : Because they do not learn enough about the importance of muscle balance in PT school ,and doctors just want to write prescription.

@parantunut "Re This video fails to address the reason for why his pelvis is out of alignment.

Cosmeticsurgery1 2 years ago
You remeber wrong: It was piriformis surgery. It would be good to see the video first and then comment.
If it has happened once, it can happen again. If it has never happened, it can still happen tomorrow. What's your point?
parantunut 2 years ago
My point is: it will be nice to clarify that this is not a curative treatment. I mean it will be nice that during the video some general info will be exposed. For example statisticaly how often rezidive can happen etc?. In my view this way you will make the video more accurate. My point was not to doubt about the treatment. Thanks for taking the time and answering
Cosmeticsurgery1 2 years ago
It is curative. Sure it can happen again like you can brake your leg many times if you keep falling down from the roof every time you go there.
"how often rezidive can happen"
My dictionary doesn't know word "rezidive".
No-one has statistics. Every human is different. And it depends what you do.
parantunut 2 years ago

Table tennis daily . . check recent advances in pelvic fascia; some news there.
bakguru1 2 years ago
I understand pubic symphysis misalignment is corrected by pushing the innominates back to alignment. That's what he did.
parantunut 2 years ago
Might happen spontaneously but unlikely. It pays to check, then use MET. If that fails, go on to more assertive moves. Pub symph acts as a lock for the pelvic ring, and force closure there needs to be modified at the instant of adjustment. cheers .
bakguru1 2 years ago
Interesting view. I have seen few experts using MET but no-one told that. Have you any studies to show?
And you don't believe pubic misalignment can be affected by moving innominates? It is quite a small joint between them and easy to affect...
parantunut 2 years ago
Learned it years ago in a workshop based on Rich Paigh. Innom manips alone are not precise enough for me in resetting the symph. MET uses the adductors to spring the symph, but only after doing the SIJ's. Always ready to test another way. Perhaps if you google migratory fascia in pub med, also look at Diane Lee's website, useful general stuff there too. Lots to read!

spondylitis...But the majority of problems dont link the SIJ....A therapist should treat and educate the patient by alleviating the MECHANICAL factors that bring the backache (from self-treatment exercises to correcting posture)...ONLY by this manner there will be a LONG-LASTING relief of pain...That is my experience in practice, and because this works almost always i dont give a damn for any "malalignment" in SIJ...just that, if u want to talk in your terms...
quorthon1940 2 years ago

The pelvis does move! and a slight unilateral shift is all it taktes.
jojosherwood 2 years ago
I am a NASM certified personal trainer and corrective exercise specialist. I know about biomechanics--the kinetic chain--muscle imbalances and ways to correct them. There may be an imbalance that makes my pelvis shift and I am working on that. When my pelvis shifts--my left leg seems up to 1" longer than my right. I am literally tilted until it's adjusted. I was told I have degenerative disk by orthopedic surgeon. Most people do to some extent. That was not the CAUSE of my pain. My SI joint was!
jojosherwood 2 years ago
well because my friend u are a "personal trainer" and a "corrwective exercise specialist" (WTF is that??lol) u have no DAMN RIGHT to speak about C-L-I-N-I-C-A-L problems!! in my profession as a physiotherapist(in clinical grounds)we have the legal RIGHT to treat patients, u dont!! So simple dude and no more talking!!
and btw i am VERY close-minded!!! So much u cant imagine!!

wow--thank you for correcting me. I am a worthless person who has never helped a person because I am not a physiotherapist who can't spell and is extremely rude. I truly feel sorry for your "patients." You my friend are truly "S-T-U-P-I-D." Go back to your cave.
jojosherwood 2 years ago
By the way--I don't "treat" patients--I help to prevent injuries by addressing muscle imbalances BEFORE they cause injury. I always refer if potential client comes to me with active pain. Before I studied for my certs (well-respected ones--look them up) I had a year of pain from impingement. 2 orthos and 2 rounds of PT did nothing to "CORRECT." I fixed myself through stretching/foam rolling pec major/minor/lats/levator scap/upper traps and strengthening rhomboids/mid traps/rotator cuff.
jojosherwood 2 years ago
fix your brain too, through stretching/strengthening,I suppose that you will find many "imbalances" in there, trust me!!!....I am going primitive again!!!
hahahaaahaa noob
quorthon1940 2 years ago
since 4 "C L I N I C A L" professionals such as yourself couldn't "fix" me and I fixed myself I am quite happy with my imbalances in my brain. You aren't worth my time.

biomechanical factors (prolonged sitting, bending, loads on flexed position, etc) and the problem then is prolonged by many pscychososial factors that will lead to CHRONICITY of back pain....The main "root" of the problem is exactly on these factors....SIJ plays minimal role on that....Though i cannot deny that SIJ pain (u call it dusfunction or whatever) can occur in certain situations....Like pregnancy, trauma (fall on buttocks, blow on the low back) or inflammatory processes like ankylosing
quorthon1940 2 years ago
You still confuse SIJ pain and dysfunction. THEY ARE NOT THE SAME!!! I DON'T CALL SIJ PAIN AS DYSFUNCTION!!
parantunut 2 years ago
u still focus on a certain field of literature....I can give u some links too by the studies i mentioned u too....In fact most of the studies that i saw dont include large samples of population and many of them are just "conclusions" from SIJ experts by applying a couple of manipulations....neither to mention that there is no FOLLOW-UP that will give us strong evidence that this theory is true...As i have said before the fact that many people suffered low-back pain is by hundred reasons from
quorthon1940 2 years ago
If you do the correction and pain is gone, fine.
If pain comes back after few days or weeks, you need to re-correction. If you don't re-correct but only follow, the results from one treatment are not that great. Most important is that people learny what is the cause of their pains and how to get rid of it.
parantunut 2 years ago
by "muscle spasms" in the back which then can affect the whole system and and....They discard EVERY different aspect in a FASCIST way!!!My point is that this MONOLITHIC manner of thinking in such a science is almost catastrophing in clinical practice...Nothing comes from ONLY one source!!The clinicial should have means for proper DIFFERENTIAL DIAGNOSIS....Open your mind, read more books and articles, reconsider some thoughts and then we talk again my friend.
quorthon1940 2 years ago
I agree. Muscles affect the whole system. Muscles are the factor that connect pelvis to shoulders, pelvis to thorax, etc.
I have read some studies about palpation and provocation tests for SIJ dysfunction, but because they believe dysfunction is pain in sij their results are useless. Some interesting stuff exists drom Cibulka, Hesch, DonTigny, Al-Elsa, LeBlanc, Schamberger and few others.
parantunut 2 years ago
the field of musculoskeletal medicine is completely DYNAMIC, and beliefs that were believed 15-20 years ago can easily be abandoned if there is a steady scientific proof via research.... i know many people like u who have similar beliefs, for example one says that all BACK PAIN caused by "Instability" in the deep muscles of the back, the other claims that BACK PAIN caused by abnormalities in cerebrospinal fluid (check the word "craniosacral therapy" to see) and the other that BACK PAIN is caused
quorthon1940 2 years ago
Yes, some experts tell one stuff and other tell other stuff. That's not news.
parantunut 2 years ago
as i see my friend u live in your own reality....Your philosophy that EVERY kind of malfunction (back pain, sciatica, neck pain, headaches, what else carpal tunnel syndrome maybe???) is caused by "PELVIC DYSFUNCTION" give me the chance to advise u that you should start looking from a UNBIASED and truly SCIENTIFIC literature about the subject we talking....When someone claiming himself as a SCIENTIST and not an "experienced chiropractor" he must be fully aware of what is new in his field of science
quorthon1940 2 years ago
Where did I say EVERY?
I have learned that scientific way is not allways the correct way. Science don't believe pelvic dysfunction exist. Science don't believe facet locks exist.
parantunut 2 years ago
Leas but no last...What i said before about the asymptomatic patients and a possible difference in mobilty i meant that simply this normal "variation" in "mobility" (i put " " because mobility is almost absent) means that this clinical sign is completely ATYPICAL and valueless in a clinical examination....I think now that i make myself clear....
quorthon1940 2 years ago
I dodn't understand.
There are studies telling fibromyalghia patients have abnormal GAIT.
There are studies that back pain patients have abnormal GAIT.
Studies tell, those athletes who have altered GAIT suffer more injuries.
etc.
parantunut 2 years ago
This patient of course it is possible to have some sort "POSTERIOR TILT" in his pelvis (lets say that it will be more pronounced one-sided.) and of course the pelvis will be dysfunctioned....But think this.....All this DYSFUNCTION is merely a R-E-S-U-L-T of what had caused the back pain in the first place... and it is the bad posture, repeated bending if we talk about this patient.....So u as a therapist will u treat the CAUSE or the RESULT???? consider that before answering....
Cheers
quorthon1940 2 years ago
No. As experts explane it, the pelvic dysfunction is the cause and pain in back, or somewhere else, is the result.
After correcting pelvis he was pain free. Like most are. Like I was.
parantunut 2 years ago
In this occasion if you target on a "dysfunctioned" pelvis, which is TRULY obvious u will make serious harm to patient....Instead u should focus what happened in LOW BACK!!! when u resolve that the whole system is getting straight...Maybe it is an extreme example, but i think u get the point....To make it more simple.....The classic presentation which is commonly seen in private practice clinic....Low back pain because of prolonged sitting or bending with often radiating pain....The same is here
quorthon1940 2 years ago
Yes, I had that. Sitting was painful for years, I had sciatica. It was because of my pelvic dysfunction. It's gone now.
parantunut 2 years ago
"Most mechanical problems in back are to due to dysfunction in pelvis"!!!! Where in the name of god u conclude that????
Have u ever wonder that the opposite is more possible to happen??? A MECHANICAL (name it disc, facet, ligament or whatever u like) LOW BACK PAIN with all that accompanies can give malfuntion to the pelvis and lower body....An example on that...Think about people with "LATERAL SHIFTS", means that because of some acute disc pathology they become "inclined" to one side...
quorthon1940 2 years ago
As a clinician i examine the SIJ lastly after i have exclude MECHANICAL BACK PAIN (Check the McKenzie books for this rationale...) then the HIP and only if those are not the source of pain (and consequently the "dysfunction" as u claim it) i would suspect the SIJ...I never trust palpation techniques but i prescribe exercises to the patient which truly "mimick" movements that u can achieve by mobilization (and not THRUST manipulation!!)....
quorthon1940 2 years ago
Many doctors diagnose the dysfunction with Fortin finger test: A patient points the location of the pain. If he/she points to SIJ area a doctor may diagnose dysfunction to that place. most doctors don't much investigate or touch the patient.
Most mechanical problems in back are due to dysfunction in pelvis. It is easier to first correct pelvis because immediately many back muscles start to relax. It is easier to open facet locks and other locks from spine after pelvic correction.
parantunut 2 years ago
Not to mention that many ASYMPTOMATIC people has "differences" in SIJ mobility...
u seem to be familiar with the term "DYSFUNCTION"....when one tissue is normal, suppose it is the sacroiliac, and i mean "normal" by the fact that it is NOT the source of PAIN, that the patient is complaining off.Why sould this tissue has "dysfunction" when it is not the source???
quorthon1940 2 years ago
Sure. Like I said: pain comes usually to somewhere else. More than 80% of all people have sufferen pains because of pelvis misalignment/twisted pelvis.
parantunut 2 years ago
I can give to u as many proofs u need about the fact that SIJ has MINIMAL movement, u just check the work from Sturesson in the literature(i can give u the exact articles if u want...) where he proved by a stereometric alanysis that 1. the movement of the ilia on the sacrum is about 1-2 mm and the most important 2. people with back pain DIDNT have HYPOMOBILE or HYPERMOBILE sacroiliac joints especially if the pain was one-sided!!!
quorthon1940 2 years ago
I agree. It has minimal movement. I haven't been telling anything else.
parantunut 2 years ago
and is entirely an "neurophysiological" phenomenon (if u are a bit aware of the -so called- ARTICULAR MECHANORECEPTORS and how they get affected by external forces and how they inhibit or exhibit pain, u wil probably see my point). Finally i want to comment that methods like palpation and "estimating" anatomic landmarks as u did on video is by far an UNRELIABLE method of examination, also proven by many clinical trials and studies....
Have a nice day
quorthon1940 2 years ago
Palpation is not good way of looking for sij pain. Palpation is a good way of looking causes of pain and stiffnes.
parantunut 2 years ago
possibility that this guy had a HIP dysfunction had u checked it??? Definitely no!! The fact that this guy has instant improvement says nothing, because it is impossible to MANIPULATE a joint (SIJ) that has 1-2 mm movement as it has been proven in the literature....U should show us a FOLLOW-UP video of your patient and see if that improvement was "permanent", which i doubt cause in fact NO movement has been made (as i stated earlier). It was just the usual effect seen after a "successory" thrust
quorthon1940 2 years ago
Cibulca has proven it moves
hip dysfunction is as common and was aslo corrected.

parantunut 2 years ago
a critical question to the guy who uploaded this....by what kind of examination u figure out that the pain is sacro-iliac related??? let me remind u (of course if u are a registered physiotherapist or chiropractor) that the sacro-iliac prevelance in back pain is 15% according to literature (see works by Scwarzer, April, Bogduk and many others..)...Only if u have differentially diagnosed that there is NOT MECHANICAL LOW BACK PAIN u can see other possible resources of pain...for example the
quorthon1940 2 years ago
15% of low back pain is in sij, but sij "dysfunction" is the cause of pain in many places from feet to neck. Usually in buttock or low back.

Sinan97082 3 years ago
Who was this guy? and is there anyone else you may know?
iasis7 3 years ago
I am afraid my father knew his name, BUT this guy died 2 years ago! But in Turkey, there are some other as i heard. But i don't know them, because sinc i have been to this old guy, i had no more pain in my back.. (lowerback - right side) again!!! AWESOME!!!
Sinan97082 3 years ago
He found a point, and then he said "oh it is non on the same line" i said that it was always like that! Than they told the others to hold me with all their strenght..and i was told to become relaxed! AND all i know is that i heard a big "KLACK" in the area where my belly is. After 20 minutes relaxing i get up and... wow i did't feel any pain or other bad things. Now 4 years later, i'm sill painfree. After this, i've made many experiences like that..OLD IST NOT BAD, STRANGE TECHNIQUES from asia!

doing any exercises to stregthen opposing muscle groups? He will surely have Psoas and gluteus med. involvement due to pelvic torsion. Look for knee addtcn on a single leg squat =gluteus medius weakness from overstretch. check for ham weakness on eccen conctraction b/l (the ability to bend the knee while moving backwards, say in a reverse lunge. a tight, weak hamstring will continue to allow the high hip to pull up in combination with quadratus lumborum tightness on the ipsilateral side.
slaterzzz 4 years ago
Something like that. It is a pity not all know that or even anything like it. Pelvis torsion creates muscle problems to piriformis, psoas, gluteus and to many others. After adjustment of pelvis the muscles have to be stretched to adapt.
Problem is that just "opening the SIJ lock" is not enough to everyone. The ilium can be lifted up some sentimeters and opening the lock doesn't move the ilium back to it's place.
And anyway opening the lock leaves SIJ very loose. It must be pushed back.
parantunut 4 years ago
whenever there is a high ilium, the L5 lumbar will rotate toward the inferior side. L4 will typically rotate toward the high side unless there is thoracic or even c-spine involvement. you should take these into consideration when addressing any SI problem. also, just stretching a muscle may lead to more problems, you should check to see if the muscle is weak because of over-stretch or over-contracted. if overstretched, strengthen (dont stretch) if over-contracted, stretch (dont strengthen).
slaterzzz 4 years ago
also, it looks by the way the therapist is "unlocking" the SIJ is by causing a myotatic reflex within the QL mm on the R. This will normally be reset when in standing position after a few steps because of weightbearing principles of tonic head/neck/pelvic reflexes that right the pts body by head and eye movements. Addressing the entire spine and chiropractically adjusting the SI, L5, L4, T13, and C1 will typically help solve the problem
slaterzzz 4 years ago
oops T12

> Addressing the entire spine and chiropractically adjusting the SI, L5, L4, T12, and C1 will typically help solve the problem
Sounds great! It is a pity that all chiros don't know that. Or perhaps they do but they don't know how to adjust them. Why that info is not published anywhere? You should make that information known to all patients!
parantunut 4 years ago
Not to say that there are not congenital and anatomical reasons for hip misalignment, but lifestyle is a major contributor and makes it worse over time.
leo5543 4 years ago
I know an athletic trainer that did a different procedure to correct the alignment of the hips and length of the legs. It involved putting the persons shoes under different positions on the back. It really helped me, but it is a procedure that must be repeated. The awkward positions we put ourselves during our daily routines and the shoes wear cause misalignment of the hips. Unless one changes his lifestyle, and properly designed shoes are made, we can not escape at least a small misalignment.
leo5543 4 years ago
Leg legth difference is usually not anatomic but a symptom of twisted pelvis. A heel lift is a way to correct "length difference", but not the cause of the "leg difference".

As a side note: at the end of the video there is a quote about physicians not being trained to find these kind of problems. Chiropractic physicians (and DOs trained in OMM) are trained to find these dysfunctions (and do so every day)

Like that patient I too hade visited many phycisians, chiropractors etc. end they did NOT know how to correct that.
Some of them believe SIJ can NOT be uplifted.

I am sorry to hear that... However, what is being shown is nothing special. It is taught very early on in DC education. At least in the USA.

Basic chiropractic theory... nothing special

I have this exact condition today after falling on my pelvis 9 years ago from a bycicle. Can this be healed in one time like in the video? Did it really straighten out for good after this visit?

I just got over a severe case of Sciatic nerve damage and the lower back all screwy. It took 1 year of hobbling.
It was terrifying (thought I'd never get better). Prior: I immersed myself into a fierce learning curve
on an edit program(8-10 hours a day-sitting). One day,
I ran around lifting a video cam (heavy w/ tripod).
I was toast. Talked to about 20 people with similar
problems...seems there was no identical remedy and the
docs all did and said diff things. Thanks for the post.
