As far as I know, as long there's banana leaf rice, it ain't a mamak stall and Mamak is an Indian Muslim. To me, places that sells banana leaf rice are just Indian Curry house restaurants.
Mamak is the shorter version of the name Mohammed. Kedai Mohammed= kedai mamak. Try saying Mohammed fast, Five times Malay style and you then get the idea.

You miss some more important criteria:
1. You have to wave like crazing to make sure the 'ane' notice you esp during peak hours.
2. Most waiters are not Malaysian. They are expatriate from India, and may not understand Malay or English.
3. The famous "Sekali or asing" question when to pay.
