Qkia has a point. According to the strategy guide you can unlock Mikey's costume by beating the Feudal Japan Shredder boss level on an S rank. Did you do that, or is that a cheat code?

The New leather head some how survived  when the celling colapsed on him. I don't know how though you see him in a cut scene on the Ninja turtles 3 Mutant Nightmare after you defeat the the evil ruler of Traxmus people. The don't explain it really.
