He looks to be supporting weight on the side bars, this is reducing the effect of traction. If his feet are hurting, he should try using boots instead of ankle clamps, as they more evently distribute the weight, reducing pressure points.
'Cold' makes sense, as blood drains out of the feet from this. Not any different from holding arms overhead for long periods.
If it is painful, it is probably good he is not fully decompressing, should fix that before letting go or applying more traction.
