supervisor on set. When everything's shot, processed and is in the cutting room prop continuity is one of the last things on an editors mind (and irreversible unless they re-shoot).

That does make sense that it would be someone on set who has the responsibility though, I should have thought of that before. Thanks for the info!

Of course it's not completely disregarded by the editor - for example the editor can check if the mistake is absent in other takes, or cut away from it. But usually they have to leave things like that in there and hope they can get people so involved in the scene that it goes unnoticed! :)
