For it to work you DON'T check off the box that says "Show a link to this page". Leave that part blank.

@deyshavu the new customize bar comes up on the far left of the screen. scroll all the way down to "pages" and the window that pops up in the video above will come up. follow the instructions in the video. type in the html code correctly in the description box and it should be fine. let me know if it works for you!

Great video, it helped me out a lot because I had been trying to do this for a while now.... The problem though is (if anybody has knowledge of this) whenever I try to make the page I get this message: "This URL is reserved for a built-in Tumblr page. Please use a different URL."
