This is a real train. It is merely filmed with a lens with a high F-number. Probably 1.8 50mm or less.

The train is indeed real, however the depth of field is not the same across the frame so the f1.8 idea doesn't hold up (and checking the source footage it was actually f5.6 !). The effect was added in post production using the 'swing tilt' process.

I think it is a combination, I know the train is a model so the guy just brought in some video of moveing trees.
