Hitting his head on the table would have made the guy dizzy at the most---I'm without a doubt that Joker intended to kill him and did so as it appeared before your eyes.

....that's just having a confident sfx man and stuntman to trust that said sfx man.....that one gag shouldn't make it any more expensive...given the range of other gags (semi truck flipping over on itself) i'd say the pencil was....cheap and easy....but as a whole you mean? (in joker voice) Yeah!

I think the pencil wasnt even there when Joker smashes his face on the desk. the editing is done where you dont see the pencil anymore once the guy walks up the the joker.
look closely, it's there at 29-30 seconds when the shot begins and it's one continues shot, no room to edit.
Now, another possibility is that the actor playing the thug is a stuntman. Given the fact that they had to make it look like they plowed his head into a table a professional would be better to use, actually, now that I say that, I bet that was a stuntman, he could have reached down and grabbed it before he got thrown down, however, his arm movement doesn't back that up...

I still cringe when I see this scene. I'm torn between whether or not I want to see the pencil in the guys eye........which is really the only way it could go through his head so cleanly.
