the worker fell simply because he failed to use appropriate PPE while testing the geronimo line. he also forgot how the brake handle works. If he had been tied off properly he would not have fallen to his death and if he had remembered how to operate the brake properly he wouldn't of fallen. this unfortunately is a very good example of how worker inattention to detail causes fatalities in the workplace. i was accross the road from this when it happened so don't tell me i am full of shit
