A universal hack for chams cant be accomblished. Its technically not possible unless you log the chams, texturecrcs or anything identifying which object is to be chamed or not.

Normally you use a combination of stride, numvert and startIndex passed to an directx api called drawIndexedPrimitive.

You could also identify the current object by a texture checksum and alot of other stuff. But there is no general method for all games at once.
