No, it's free to start. I don't know about actually publishing something, but you make your own stuff for free and download the engine for free. But yes, after a certain amount of profit you have to pay a percentage. As I said, I'm not sure about licensing other than that.

If you want to sell your game though, then you have to pay $99 and when you make $5000, 25% goes to Unreal. Great deal, if you ask me.

The same as with any engine. You use external programs usually to make objects and textures, like Gimp and Blender if you want to stay free.
