No way to tell from that angle if it was definitely an elbow. Kovalev obviously should have been suspended for his huge elbows on Hollweg and Tucker in years past, but this hit on a Gange with his head down is much more borderline.
I agree with you in the sense that it was not really easy to tell whether it was an elbow or not and that Kovalev should've been suspended for those two elbows but there was obviously intent to injure and Kovalev appeared to be head hunting (which he has been guilty of several times before) and according to Campbell, the league wants to get rid of hits like this. If that is their mentality than Kovalev should've been given a 1 game suspension for this hit and 5 for each of the other hits.
Well, from watching it over ten times, i've gathered this:
1.) Kovalev had all the time to get out of the way due to his rate of speed (and considering the puck was no where near Gagne when the hit was made)
2.) Gagne had his head somewhat down when following the puck with his eyes, but still, as said above, he should be able to be in the position he is in, since he doesn't have the puck.
3.) Although it was a shot to the head, I can't really tell whether it was with the elbow or shoulder b/c..

4.)I don't know if this was done with intent to hurt Gagne, but Kovalev does get preferential treatment from the league, given his league status (along with Malkin and all star players). This isn't the first time something like this has happened, with Alex involved.
His elbow could have easily hit Gagne, though, given the fact that Gagne was in a bending-over position, somewhat parallel to Kovalev's elbow, so that could possibly have been a way for Kovalev to sneak the elbow in.
They'll do nothing, they haven't done anything in favor of the Flyers before, why now? Simon Gagne is just as talented as any other superstar, but why would the NHL give a suspension to the alleged leading all star game votes leader? Kovalev doesn't even belong in the all star game anyways the way he's been playing.
