I don't think it's about child abuse. I think the first two children had events beyond their control bring a negative impact (the boy was in a car accident, the girl had birthmarks covering her body...I've met a girl with that exact problem, and she felt she couldn't wear dresses because of it), but they had love and support from their parents. The last boy had no obvious physical detriment, but was clearly at odds with his parents expectations and so never felt genuinely loved or accepted.
