@RUlegaleagle: It was initially created in the big bang. It is slowly winding it's way from hydrogen towards iron, its end point. Intergalactic space, while quite thin, is not totally empty, and space is BIG - very BIG. Interstellar space, where stars are born, is a lot more crowded, but is still pretty empty from our perspective.
