He is able to shield them with his forearms and elbows. For instance Ricky Hatton who like Ortiz led with his head a lot. To stop from getting butted, he struck him with lead rights and check hooks and extended his arms to prevent him from getting into his chest. Thats why Ortiz had to resort to a head butt!
